Your primary focus will be to support individuals and families who are facing the challenges that arise from a diagnosis that is terminal and at the end of life. Responsibilities:
- The weekend schedule begins at 5 pm on Friday and ends at 8:30 am Monday morning.
- Assist the RN in carrying out the plan of care and PRN visits.
- Under the direction of the RN triage, supplements the nursing care needs of the patient as provided by the RN and as specified in the POC.
- Assist the patient in learning appropriate self-care techniques.
- Complete appropriate clinical and progress notes, documenting outcomes of interventions.
- Assess patient/family response to care, ensuring communication of information to appropriate team members.
Qualifications:
- Current State Licensed Vocational Nurse.
- Current State Driver’s License, reliable transportation, and provide ongoing valid and current auto liability insurance.
- Graduate of an accredited school of vocational nursing.
- Minimum of one year of hospice experience as a Licensed Vocational Nurse.
- Valid BLS.
- Successful completion of Background screening including employment verification.
