Our census is increasing and we are currently searching for a compassionate, empathetic, and experienced Full-Time Social Worker to join our team.

Responsibilities:

Serve as a patient, family, and team advocate.
Clearly and effectively communicate the value of hospice & palliative care service to other team members, patients and families.
Have the desire to work with those at the end of life.
Provide professional psychosocial and emotional support.
Serve as a resource for the medical team.
Participate in the patient care planning process and promote coordination of patient care.

Qualifications:

Education: Must have an active Social Work license from an accredited school of Social Work.
LMSW or LCSW required
At least two years experience in a clinical care setting that may include: hospice, hospital, long-term care, or clinic settings.
Great interpersonal skills, both verbal and written.
Familiarity with an EMR (Electronic Medical Record).
